PEN-Plus: Advancing NCD Care Across Africa

Non-communicable diseases (NCDs) are becoming a growing health crisis in the WHO African Region. A recent report by the World Health Organization highlights the success of the PEN-Plus approach, a strategy designed to decentralise treatment for severe NCDs and improve access to care in underserved communities. Since its adoption, 20 African countries have enhanced service delivery for conditions such as heart disease, diabetes, and cancer.

NCDs are chronic conditions that develop over time and are not spread by infection. They are driven by genetic, environmental, and lifestyle factors. Globally, NCDs account for 71% of all deaths. In Africa, they cause between 27% and 88% of total mortality, depending on the country.

The PEN-Plus model builds on the WHO’s essential NCD intervention package by focusing on care at the primary healthcare level. Developed in Rwanda, it targets people living in extreme poverty and trains healthcare workers to provide diagnosis, treatment, and psychosocial support.

As of January 2025, over 15,000 people have received treatment through PEN-Plus. Countries like Malawi and Liberia have made notable progress, with Malawi training more than 440 clinicians and launching services at six district hospitals.

However, many still lack access to NCD care. The WHO urges continued investment to scale up PEN-Plus and build sustainable health systems that ensure no one is left behind.
