A vision of chaos unfolding and a lack of proper regulations led to the preventable tragedy that unfolded at a Goa temple, killing 14 women and children, a fact-finding committee has determined.

A reported crowd of around 700 people had gathered at the temple, dedicated to Lord Ganesha, on the banks of the Mandovi River, where a ritual to grant wishes was to take place. The panel, constituted by the state government, cited an absence of proper management and regulation of the event. On the other hand, temple management blamed the tragedy on zealous devotees who rushed the venue to get front-row views of the rituals.

The committee, headed by former judge Jaidev Georgie, submitted its report to Chief Minister Pramod Sawant on June 8 and detailed the tragic missteps that led to the crushing deaths of nine women and five children. From the collection of the fact-finding committee, it is evident that the tragedy was "entirely preventable," and the committee hopes that the government will analyze the report and put in place stringent measures to ensure that such a tragedy is never repeated, Georgie stated when handing over the report.

The committee also recommended that responsible officials should be held accountable for their negligence, which led to the deaths of the women and children.

As the inquiry concludes, the CM has ordered a mandatory audit of all religious institutions to ensure they adhere to safety standards and control measures to prevent any such events moving forward.
