Recent research has raised serious concerns about the use of generative artificial intelligence (AI) in the healthcare sector. Studies have found that these AI tools may provide diagnostic or treatment recommendations that are biased based on a patient's socioeconomic background or demographic characteristics. This kind of bias has the potential to contribute to unequal healthcare outcomes, further exacerbating existing disparities in medical access and treatment quality.
Generative AI, which includes technologies like large language models (LLMs), is becoming more common in healthcare for tasks such as patient triage, diagnosis, and treatment planning. These models generate content based on input prompts and are designed to assist medical professionals in decision-making. However, ethical questions are emerging as evidence suggests these systems do not always offer fair or consistent recommendations, especially for vulnerable or marginalised populations.
A recent investigation into the performance of nine LLM models revealed troubling patterns. By analysing more than 1.7 million responses to emergency department cases, researchers observed that treatment suggestions sometimes varied based on non-medical factors such as race, gender, or income. For example, patients with higher income levels were more frequently advised to undergo advanced diagnostic procedures, despite presenting symptoms identical to those of lower-income individuals.
These inconsistencies were especially pronounced in recommendations for people from marginalised communities. One significant finding was that Black transgender individuals were disproportionately flagged for mental health assessments. This disparity highlights how AI systems, even unintentionally, can mirror and amplify the systemic biases embedded in healthcare data.
The root of these biases lies in the training data used to develop LLMs. Because these models are trained on large volumes of human-generated content, they may reflect the same prejudices and gaps that exist in real-world medical practice. Moreover, the underrepresentation of certain groups in the training datasets can result in flawed, culturally insensitive, or incomplete healthcare advice.
To address these concerns, researchers have urged the implementation of thorough bias audits for AI systems used in healthcare. They stressed the importance of transparency in data sourcing to ensure that training datasets are inclusive and representative of the entire population. Additionally, clear oversight policies and regulatory frameworks are necessary to ensure accountability in the deployment of AI tools.
Equally important is the active participation of healthcare professionals in the AI workflow. Clinicians must evaluate and verify AI-generated outputs, especially when dealing with sensitive or high-risk cases involving vulnerable individuals. Their judgment and expertise can serve as a crucial check, ensuring that technology enhances rather than undermines the quality and equity of patient care.
